For Close Account request, please complete and print the form below

For Giro Application form because of bank account migration, please complete the form below. This is only for all existing customers who are currently paying via Giro.

Please allow your bank about 4 to 6 weeks for the processing and approving of your account. In the meantime, we request you to make your payments by cheque or cash.
